Community Habilitation Assistant
Provide individualized services that support learning and assistance in any of the following areas: self-care, sensory-motor development, socialization, daily living skills, communication, community living, and social skills in accordance with the prescribed individual and agency goals. The primary focus of training will be in integrated community settings. The nature of this position makes it necessary to work flexible hours. Supervision and assistance will be provided to consumers during breaks and lunch. Responsible for direct supervision of consumers. Direct Support Professional
The Direct Support Professional will provide integrated living support services and supervision to individuals to assist them in residing in the community. May also provide supervision and care for a consumer with physical handicaps, including, but not limited to, assisting to transfer, bathe and toilet. Direct Support Professional Relief
